Hours, Salary and Location:
Salary £28,840 FTE per annum
Permanent Contract
37.5 hours per week, Mon – Fri (9-5); part-time applications will be considered
Location – based in Buckinghamshire. County-wide travel is an expectation of this post.
Pension, with employer’s contribution rising to 5% on completion of probationary period.
Cost of living increases are approved annually by the Board of Trustees subject to sufficient funding being available.

Purpose:
Elmore Community Services is a leading expert in working with individuals with complex needs and mental ill health who have fallen through the gaps in existing services. Our assertive outreach model and supportive team approach help people improve their lives and integrate into the community.
Our High-Intensity Need and Floating Support Outreach Workers will work alongside key local services in Bucks to provide community outreach and interventions to individuals who have been identified as needing additional support and who are making high-frequency use of these services due to ongoing unmet needs. The aim of the service is to identify clients’ needs, support them in making steps to achieve their goals or access alternative, longer-term support, and enable them to make more appropriate use of the services they are open to. The HIN component will be short-term and involve weekly contact with clients; some individuals will then be offered longer-term floating support and will continue to be seen fortnightly for up to 2 years.
Key Responsibilities
Provide a high-quality support service to clients:
To manage a caseload of allocated clients and liaise with partner organisations to identify which individuals will benefit from a brief intervention.
To provide creative and flexible brief interventions to enable these individuals to develop self-management strategies and produce plans to effectively manage their mental health crisis and access suitable support in the community.
Develop and maintain a good partnership with other services by regularly attending and participating in team meetings and other meetings as required.
To actively manage and update clients’ risk assessments in line with Elmore’s risk assessment policy and procedure and effectively manage safeguarding concerns.
To act as an advocate for clients to ensure that their voices are heard
To maintain a wide knowledge of external multi-agency services to facilitate and support client access to a range of agencies
Contribute to the high performance of the wider Elmore team:
To work within the team approach and to be a supportive and effective part of the team.
To attend and participate in team meetings, planning meetings and policy reviews.
To work as part of a multi-agency team, liaise with partner services, and provide data regularly to ensure accountability and evidence of client engagement and activity.
To maintain clear and accurate records and complete relevant monitoring relating to the clients you support to fulfil contractual needs.
To actively promote Elmore to stakeholders and represent Elmore at external networking meetings and multi-agency meetings regarding clients.
To work within Elmore’s policies and procedures, including Health and Safety regulations, Equality and Diversity and Safeguarding.
To undertake any other relevant task as defined by your line manager in relation to the caseworker role.
Personal development
Be proactive in reviewing and evaluating personal performance and identifying areas for improvement and development.
To keep current with relevant legislation that might affect clients (including mental health, benefits, housing and criminal justice).
To attend and actively participate in supervision sessions.
To attend and complete relevant training as agreed by your line manager.
Person Specification
Experience:
At least 3 years’ experience of working with people with support needs related to mental health, social isolation, drug use, alcohol misuse, homelessness, offending behaviour, antisocial behaviour, or challenging behaviour (essential)
Experience in providing brief interventions to individuals with complex and/or multiple needs (desirable)
Experience of working in partnership with other agencies and liaising with statutory and voluntary sector care providers (essential)
Experience of working independently and managing own caseload (essential)
Experience of risk assessment and management and safety planning (essential)
Experience of using a case management database and keeping accurate case records (essential)
Experience of working in an often-demanding environment as part of a team and coping effectively (desirable)
Skills and Abilities:
A clear understanding of the issues faced by people with complex support needs, particularly those which might arise when dealing with clients in a community setting (essential)
Knowledge of adult and child safeguarding issues and the roles and responsibilities of relevant agencies (essential)
Ability to respond flexibly, creatively and persistently to a range of challenging casework issues (essential)
Ability to build effective relationships with clients, especially clients who are difficult to engage, providing support respectfully and without creating dependency (essential)
Knowledge of appropriate services that Elmore clients may need support to access (desirable)
Strong written and verbal communication skills (essential)
Ability to use Microsoft Word, Outlook and Excel and a readiness to learn new ICT skills (desirable)
An understanding of relevant legislation that affects clients, including Homelessness, the Mental Health Act and welfare benefits (desirable)
Car driver with full licence and access to a vehicle for work use (desirable)
