Making a Referral
If you want to refer yourself, please telephone Elmore between 10am and 12.30pm to speak to a worker. PLEASE NOTE – the office is currently closed; if you leave a voice message someone will call you back.
If you want to refer someone else, please complete the Common Referral Form and send this via email or post.
There is also a Guide to help you complete the referral form.
Elmore will respond to referrals as soon as possible. We do currently have a long waiting list, of at least 12 months from receipt of referral to assessment.
We are also happy to speak to other professionals about potential referrals, and sources of support which may be useful for their clients.
